USCIS的信中是這樣的：

Please fax employment documents to the following number: 816-251-2290 Attn: LA0867, Div 5, Team 2. Please include this notice with your fax.
我的情況是原來的工作簽證1月1日到期，公司已經terminate我了，但是又給了2個禮拜的unpaid leave如果能搞定EAD還可以回去。我之前打電話的時候也選擇了expedite request的原因是financial loss. 我想知道這employment document裏麵應該包含啥內容？應該讓我的manager 還是HR來簽字？
