5. I-693 which is completed by a Dr. Find a Dr.
6.Form G-325A Biographic info
7.I-864 with the support documents (same as the one for you parent).
8.Evidence of eligibility (primary beneficiary's green card/immigrant visa?)
-Yes (both sides of the green card).
9. The copy of your I-20 with F-1 I-94.
rejection notice上寫， based on the inforamtion you provided, your priority date could not be established. Please resubmit your application with proper document.
The follwoing documents may be provided:
Your original Form I-130(petition for alien relative) if you are filing concurrently or
a copy of your form i-797(Notice of action) if the I-130, ...petition has already been filed/approved or
other evidence that an immigrantion visa petition has been filed/approved on your behalf.
請問我需要補什麽材料？
我還需要重新填寫之前寄出去的材料嗎？
之前寄出去的支票也已退回，請問需要再開一張支票嗎？
另外，我發現我的EAD上有USCIS #，這個#在和EAD attach的紙上寫了A#，那麽我是否需要填寫這個#在所有要求A#的表上？
