-Yes. The visa stamp is the one you applied for in the US Consulate (on your passport), so you need to provide the cop[y of it. Also, every time when you entered US, the CBP officer should have stamped your passport (unless you entered form Mexico/Canada), so provide the copy of it. Also, download the I-94 of your last entry online and print it out and provide it at this website:

Also provide the copies of all I-94s you got within US since your last entry (e.g. the I-94 by status change or extension, if any).
