"Form DS-3053" means "Statement of Consent or Special Circumstance for Minor's Passport". So, your spouse just fill the form following the instructions at the link I cited about, and go to the US Consulate to sigh it. I have not seen there is 第5項 on Form DS-3053. Maybe you got a different form?
