我們的排期終於到了，現在要去體檢，請問8老師：

1。我們需要帶什麽表格去體檢？
-I-693.
2。醫生會給我們一個體檢報告麽？
-Yes, the Dr. will complete the I-693 with the physical exam results.

主，副申請人的體檢報告是分開的麽？
-Yes. Each person has own personal report.

這個體檢報告是要和485表格一起寄走麽？
-Yes.

3。我曾經在國內打過疫苗，但是那個疫苗記錄本找不到了，需要重新打疫苗麽？
-Probably need to do it again because you don't have the record.
