請教 8 老師 關於 I -765 Form 和 I - 131 FORM 填寫

1. EAD 和 AP COMBO CARD 即將到期，再填寫I-765 表時，是選 I am applying for  X Renewal of my permission to accept employment (attach a copy of your previous employment authorization document) 還是 Permission to accept employment？

2. H4 最近（12/10/2014） 有 COMBO CARD 入境，護照上蓋章 paroled, until 12/09/15; Purpose, AOS.
這樣情況下，H4的 I-131 表的 CLASS OF ADMISSION 是H4 嗎？I-765 表的 Status of Last Entry 和 Current Immigration Status 還是 H4 嗎？如果不是，應該怎樣填了？
