questions of immi to canada

I submitted my application on march 2004 and recieved second letter today, they ask me to do medical exam and get police record. The problem is I just got my newborn baby's Birth Certificate yesterday. Dose he also need to do the medical exam?
in the second letter package there is no medical exam form(with photos) for him, do I need sent his photo to the bufflo office so that they can create a form for him? For Additional family member form, is it OK that I add my newbaby's info below the other child's ? What else form(s) need I to refill? I am mad about my situation and don't know what to do, I appreciate if anyone could give me a hint.
plus, do I need to pay when I require the blank fingerprint form FBI and how much?
