收到移民局REF letter for medical examination 要求 submit a properly completed and signed Form I-693. If you have already submittted Form I-693, either all pages of Form I-693 are not present or all the pages are not of the same revision date.

我隨I-485遞交了體檢表。檢查了一下醫生給的複印件沒發現什麽問題。原件也是在醫生處看著醫生簽封的。有什麽渠道可以問移民局到底是體檢表哪個地方出了問題嗎？

就怕再找醫生做一份還是一樣的。
 -Probably the Dr. made mistake. Just show the RFE to the Dr. and he/she will give you a complete/correct copy.
