I just started my full time job a week ago, and the annual income is just over the 125% of proverty line a little bit, at the same time, I still have my small business by the side, and I don't want to use my spouse's income( No 864A for my spouse).

Here are my questions for 864:

I check Part6 #1 , and fill  #1a for my fulltime job,
 

I check Part6 #2, and fill #3a for my own business? Or leave it blank? Actually it still operates.
-If you get income from your own business and you want o use it for the support, then fill it there, otherwise, leave it blank.
 

 

Part 6 #5 fill in $my new annual income, right? -Yes.

how about  #9c, same as #5 $my new annual income?
-No. it is irrelevant to you and just leave it blank.
 

I don't have the number for my small business profit this year yet.
 

 

#10, fill in our joint tax income or still $my new annual income? I don't have paytub, only can have employment letter from employor now.
-It is your personal annual income (same as #5) because you do not use your spouse's income.

 

For the federal tax return info:
 

I don't have w2 last year, and my incomes from my small business were pretty low last few years, should I provide last year tax return form only or last three years? 
-Provide the copy of joint-tax return of 2012.
should I mark #13? how about #13a, #13b, #13c fill or leave blank?
-Fill it and if you did not have income, put "0" there.

 

the copy of tax return form is fine or better order tax transcript?
-Doesn't matter.

Last question is, if I use our joint income as MY Current Annual Household Income (#10), that means I use my spouse income, and Spouse has to file 864A, right?\
-Yes. Then, you also need to fill your spouse income in "Personal 1" part (6.a to c.) and check "11".
