回複：急，緊張。。。公司收到DOL perm certified的regular mail，但是沒有找到9089表格？？？

今天早上，公司收到了DOL perm certified的regular mail，信上說，FORM 9089 is enclosed。 但是我們翻遍了整個信封，什麽都沒有，就兩張紙；一個是說地址，如果寄錯了，要return；另一個就是說dol certified perm，然後9089 enclosed。

但是沒有任何form 9089在信封裏。我打電話給律師，律師。。。在度假。。。下星期才回。

有經驗的，大家說說看，是怎麽回事啊？
-It seems DOL forgot to include the certified Form 9089 in the mail. Ask your HR/lawyer to contact DOL.
