The notice said 'The underlying petition indicated is ineligible'

-That means your wife is not qualified for the I-485 filing. It seems the IO made a mistake to reject your wife's I-485 and it is not uncommon at all.  So, just respond to the rejection that your wife's I-485 filing is based on your pending I-485 as your derivative status. Then, mail the whole original documents (not need to fill new forms) back to the address where the original application was return from. Put the cover letter on the top of the documents, followed by the Rejection Notice, your wife I-485 form, the copy of your I-485 receipt and the copy of your I-140 approval notice.
