DS-3032這個表格，是我填寫。我是我媽填寫[暫時沒聽說我媽那邊有收到這個表格]，表格是不是要受益人[我媽媽]簽字。如果需要她簽字。

是不是要把這份表格寄回中國。還是說在網上下載一份在我媽直接打印出來填一下寄出去就行。
