For EAD:

B1. Form I-765 for EAD renewal

B2. Copy of current EAD/AP combo card

B3. Copy of I-485 receipt notice (looks like not required)

B4. Two 2" x 2" photos (name on back)

 

For AP: 

C1. Form I-131 for AP renewal

C2. Copy of current EAD/AP combo card

C3. Copy of I-485 receipt notice 

(looks like not required

C4. Explanation why AP is needed

C5. Two 2" x 2" photos (name on back) 

C6. Copy of Passport (Biographical Page)

(looks like not required)

C7. Copy of I-797A approval notice for new H-1B (not sure)

C8. Copy of Form I-94 (Front and Back) of last entry 

(not sure)

C9. Copy of H-1B Visa with Entry Stamp of last entry 

(not sure)
