Is there any period of time in the US that is not covered by DS-2019 or IAP-66 form"
After my J-1 program finished, I re-entered US with F-2, then H1-B. DOES this question ask me to list the days after J-1 period?
-Correct (F2 to H1). But you (J2 holder) can not say anything about the J1 waiver, and just put your wife (J1) information there.

Normally, what should be included in this statement?
-such as you get the no objection letter from Chinese Government (Embassy), etc.
Since J-1 already fulfiled the 2 year HRR (want to apply J-1 waiver for J-2) , what should I say in the statement?
-Do you want your wife to apply for the J1 waiver, or you want to apply for the J1 waiver independently? If J-1 waiver, your wife does not need to say anything about that she has satistied the two-year home residency requirements (assume she has not stayed in China for two years yet), otherwise she may not get the J-1 waiver.
