皮老師，8老師 a couple questions about I-485 filing

皮老師，8老師,

I have been reading almost all your posts and articles, your professional advice help a lot.

May I ask a couple of 485 filing questions, my PD will be current on 02/01/2012. 

1. I do have an A# on my i-140 (EB2 category) approval notice, do I need to enter it to my 485 decuments. Because 皮老師suggested we leave it blank. 

2. My wife and my daughter will also file 485 as my dependents, do I need to prepare I-134 form for them, if yes, together or individually.
