Confused: how many pieces of photos required

1. Totally for I-485, I-765 (EAD), I-131(AP) combined application?
2 or 6?

2. Except signatures required, should I put "Initial" on each page of each documents?

3. Is it Ok for a Birth Certificate not issued birth place? For example, born in Shanghai. But Birth Certificate issued in Beijing. My all residence proof was transferred to Beijing when went to Univ.
