I-131,part I 3: Class of admission: what should I fill?

-check your I-94 you got at th eport of entry for your last entry to US, e.g. B2, H1, etc. It should be the same as your visa for your last entry such as B2, H1, etc.

Part II: which one should I select?

-Select "d".
