Items 4 and 1 are two-way policy regarding working and leaving the company. There is nothing wrong about it. Actually, even in big company your supervisor can create a big case about your mistake then lead to bad results.

Item 3 is benefits issue, I guess your employer just want to cut cost. If you work for bigger company with better benefits, you'll get pais sick days off.
