I think I need file I765 with I485 to ensure I can work legally in US if I find a job later. ( I am now unemplayeed, so I assume my TN is not valid anymore even thought it expires on 06/15/2012)

So, here are the questions re I765

Q11, Have you ever before applied for EAD?

Should I answer "yes" since I did apply OPT in 06/2008, if so, which office should use, the school handled it. should I just put the city of the school? and I did recieved the OPT, what type of document I need to attach? I only recieved OPT card, and what if I cannot find it anymore? (it expired in 06/2009)

Q15, what should I put as "current immigrant status" since I am unemplyed now, can I still use TN/worker as my current immigrant status?

Q16, I think I should put as (C)(9) as eligibility since I am filing I765 with I485, am I correct?
