1. Please submit proof of your continuous employment authorization in the US from 2010 through 2011.
-If your spouse has a job, just provide the H1 approval notice with the H1 I-94 during this period of time. if do not have a job, just say he is a dependent with H4 status, etc.
2. Please provide a signed request to change your preference.
