Eb1A applied and ready for submitting I485, but having questions

1. First question is about part 11: have you ever before applied for employment authorization from USCIS?

Should I put all granted casees there or only the last approved case?

2. Part 16: Go to Part 2 of the instructions, eligibility categories. In the space below, place the letter and number of the category you selected from the instructions.

After checking the instruction of I-765, completely confused and need help from here. (We came to US 1 and half year with H1B and changed H1B two times here, but never applied for EAD before)
