I forgot sumbit I-94 when I left USA. Should I mail it back? What is the address I should mail. Should I mention the reason, the flight No and the date I left US while I mail it?

-If you will stay in China for short time, then show the CBP officer the boarding pass(for the plane left US)  the stamped passport (with date for her entrance to China) to the CP officer at the port of entry when you come back if they ask you a question.

If you will stay in China for long period, such as over 3 months, then make a copy of boarding pass, the I-94, passport with entrance date to China, and a letter for explanation to the following address:
