what to fill up on I94

my mom is coming to chicago this weekend. she can't speak English at all. when we came over to u.s, we need to fill up I-94 form on the airplane. Anyone will kind and tell me what information she will fill up besides birthday , name? I need to write them down for her in both English and chinese, so they won't make any mistake. appreciated it.
