二、所需材料：
4、在美國出生的兒童，請提供出生證明、父母中國護照、有效美國簽證原件及複印件；（如兒童居住在我館領區內較遠的城市，不便親自到館，可由父母一方到館代為遞交申請。除上述材料外，還需提供：1、經notary public公證的“兒童申請旅行證的照片為兒童近期照片”的公證書；2、經notary public公證的父母未到館一方授權並同意到館一方辦理旅行證的委托書。）
