when I was asked to extend my J1 visa, people sent me an instruction about this but in PDF file, si I can not put it here. For less than 6 months extension no fee and letter were requested, but for more than 6 month and up to 2 years extension of beyond J1 3-year limit, 198$ and your and your boss' letters for why the extension is needed shound be provided and the extenion usually will be easily approved.
