Question for Applying HK Visa or Travel Entry Permit

Is there anyone did apply for HK visa or HK Travel Entry Permit as a Chinese who lives in US and holds P.R.China passport before? Please share your experience or if there is anyone else you know had this experience. What kind of documents needed for applying HK visa or Travel Entry Permit?
