As for 2 & 3, I don't think it is good to use the same letters. I mean that you'd better get the letters from department and your supervisor, which are special for your English proficiency. These are different from the reference letter from your supervisor.
In my case, I had three letters special for my English proficiency.
1. the letter from myself. You had it already.
2. From the department. This letter may mention that what your TOEFL score was when you applied for your PHD program. And, in your university, all courses are taught by English. This is not nonsense. This is an official certificate. This letter is no relation with that showing your status and financial support.
3. From your supervisor. This letter will show that how you use English for projects, group meetings and so on. This is also different from the reference letter. You'd better don't conbine them as just one letter, cause the goals are different. It seems a little bit stupid, but it is a rule.
So, provide these three letters SPECIAL for your English proficiency.
