Thank you for your reply. Yes, you are right, but in my case there is no I-94 card attached on the bottom of approved notice, I do not know why. I called the agency and was told that I need to pay $195 for the duplicate of the approved notice from USINS. I just want to know if old I94 card is necssary for the next extend of H4. By the way, I just concern the duplicate of my current approved notice could be the same one as I got so far.
