請問大夥兒 那個 I-751的 affidavits

我在準備I-751的材料，看到填表說明書上說要有affidavits sworn to or affirmed by at least two people ... 心裏挺疑問的，想問問大家：

1、這個 affidavits 是必須要提供的嗎？

2、如果這個 affidavits 是必須的，大家有誰有這個文件的“模板”或者 “格式” 嗎？文件要有落款簽名嗎？ 這個文件要拿去公證嗎？
