Question#2: on page 1, under C
14. If your relative is currently in the U.S. , complete the following:
He or she arrived as a:
Arrival/Departure Record (I-94)
Date Authorized stay expired or will expire, as shown on Form I-94 or I-95
15. Name and address of present employer ……….
16. have your relative even been under immigration proceedings?
My dad is in China now, may I "N/A" all the above questions? Or should I still fill out when he visited U.S. last time and when he left before the expiration date on I-94?
