Please Help. Can I Renew H4 by Myself?

My current H4 will expire on 12/8/09, can I apply for extension by myself next Monday 12/7/09?

If I can do it by myself, how many documents should I submit to USCIS? Please give me a list. I am living in Chicago, Illinois, which service center should I send the application to?
