Just go to USCIS website to download the Form G-325A for free: http://www.uscis.gov/files/form/g-325a.pdf

The new version of this form is only one page because USCIS now usually scan your file into their computer system so that each related department within USCIS can reach your file. But some time ago, they had to send the hard copies to different departements and that was why they asked 4 copies of this form.
