PJiang and other daxias: please help me to fill I-539 form

Hi, PJiang and Other daxia
I fill the i-539 form to change status from h1b to h4. I have several questions:
1) Part4, 3g: "Have you,or any other person included in this application, been employed in the United States since last admitted or granted an extension or change of status?"
I answer "Yes". Is it right?
And also it need provide a description "whether the employment was specifically authorized by USCIS." What is the answer for this question?

2) Do you have a checklist for my case(H1b -> h4).
My checklist: H1b notice and I-94 copy (both me and my wife)
Marry certificate
Passport copy (only me)
Do I need provide the last 3 paystubs of mine and my wife.
