請教I-485

請幫忙：
最近，I-140 of EB1A被批準了。準備自己辦身份調整。但腦子是一片空白。哪位能指導兩句，不勝感激。先問幾個基本問題，
（1）全家每人各填一張I-485，G-325A？
（2）因為是EB1A，Employment letter還需要嗎？
（3）如何Medical examination？在哪家醫院（南加州）？
