Thanks for your response. It explicitly asked for attached gold sheet. Called USCIS - the IO said a new RFE will be mailed. I just replied with a cover letter anyway. will probably mail another copy when that new RFE comes.

P.S. USCIS sent my RFE to another person (attached to her files), who kindly fowarded to me.
