請問PJANG等大俠：什麽是Golden Sheet?

收到USCIS的RFE，要我們補交Medical Examination Report。我老婆在交485時懷孕，沒有做TB，這個要求正常。不過在郵件裏有這麽一段話：

This office is unable to complete the processing of your application without further information. Please read and comply with the request below, then submit the evidence to the above the address. Include a copy of this letter and place the attached golden sheet on top of your documents.

我就收到一封信，不知道Golden Sheet是啥？請指教。
