移民局要求寫criminal history affidavit,怎麽寫?

婚姻綠卡去掉condition,兩次Fingerprint都不行,移民局要求寫own signed criminal history affidavit,supported by police clearances.Criminal history affidavit怎麽寫?應包括什麽方麵的內容?有什麽網站可以看到criminal history affidavit模板嗎?主要是以前沒寫過,怕寫的不全或不合要求.
