You only need to do any one of the followings:
1. Contact the port of Entry and go there to fix it;
2. Contact the USCIS local office in your area and go there to fix it;
3. File an H4 renewal application to USCIS to extend your H4 status before it expires;
4. Go out of U.S. and enter U.S. again to get a new I-94 within the H4 visa valid period.
