Yes, 4 copies of G-325 for your mom, so that

each department it gets by keeps one copy. If you had the carboned forms from the INS, each department is more clearly stated on the form. I don't know what those departments are. Just do as asked.

If you download the form G-325a, it already has 4 copies. Fill out the first page, your input is copied onto the rest 3 pages. Then just print the form out, and send all 4 copies, you don't even need photocopying.
