請問有誰申請過HARDSHIP 豁免或者從美國以外申請過豁免嗎?

For certain exchange visitors (J-1 and J-2 visas) and their families to apply for a waiver of the requirement that they reside outside the United States for at least two years prior to returning or leaving the United States.

Where to File :
If you are in the United States, submit the application to the Service Center with jurisdiction over your place of residence.

從上麵那段話,似乎可以從離開美國後申請.
我想了解如何申請, 至於符不符合HARDSHIP, 我有其他方麵材料(很多份美國醫生檢查報告和學校特殊教育報告).
