Which h-1b expiration date is right

Hi, All:

My h-1b begining date is 11/4 six years ago. I have one h-1b extension to 9/30/2008. at the fifth year, I tranfered to another compnay, new h-1b extension date is 7/30/2009 (first: my lawyer got this wrong, he thought I did not get any extension yet, so he filed an extension date beyond my 6-year limit. Second: USCIS approved this).
So my question is: which date is right?
9/30/2008?
11/4/2008?
7/30/2009 (Can I work to that approved date by USCIS? that will be beyond 6-year)
