1).Can research fellow satisfy "permanent position"?
-Yes, if your financial support is continuous. Just ask your Dean to write a letter indicating your current position is permanent one ( Do not say you are a postdoc, but research sciecntist,research follew, etc.) and that is all! USCIS will not come to your school to do detailed investigation for this kind of thing.

2). I provided my medical examination in 2005 when I ted my I-485, now they asked to provide medical examination again. My wife is pregnant and can not . do the examination now. What should I do?
-Just do the exam for you personal. Your wife can do some part of the exam and the Dr. will indicate your wife's situation in the exam report, and this will make sure her I-485 not be denied. After your wife has baby, she can do the entire exam later.
