-Yes,if you entered U.S with J visa for the last entry.

Line 15, "Current immigration status" should be pending AOS or H1?if the dependent is H4, the "current immigration status" should be pending AOS, visitor or H4?
-H-1; and H-4 (for dependent)

Should I enclose the J waiver approval notice with I-176?
-Not required for EAD, but for I-485.
