9. Copy of I-94 card
10. Copy of passport with visa page
11. Copies of 3 most recent pay stubs
12. Educational Evaluation Report
13. Copies of Diplomas of B.S., M.S., and Ph.D.
14. Copies of transcripts
15. Copy of the Offer Letter
H-4 application of dependants
16. Form I-539
17. Copies of all previous approval notices from INS
18. Copies of passports with Visa pages and I-94 cards
19. Copy of Notary certificate of marriage
20. Copy of Notary certificate of birth
