Urgent Question about e-filed I140 form

I completed I140 on line, However one question in the pdf form generated by
the system appears wierd.

"Has any immigrant visa petition ever been filed by or on behalf of this
person?"

I remember I answered yes and provided the case # etc, but in the pdf I-140
form generated both "No" and "Yes-attach an explanation" were checked. Is
this normal? Anyone has this experience?

Do I need to attach a separate sheet explaining my previous case in the
supporting documentation and mail to them? Thanks a lot!
