請教Pijian and 各位高手 :question about DIY H1 extension

準備自己辦H1 extension,
1，receipt notice and approval notice 是寄給老板還是寄給本人？
2，I-129 form, Part 7 "signature of person preparing form, if other than above."是否空著不填？
