請大家賜教:EB1B申請需要把博士期間的獎學金寫上嗎?

在中國博士畢業時得到的聯邦醫學教育獎學金金獎，因為也沒有其他獎，在申請EB1b時，有沒有必要寫上去?這樣會導致補件嗎?
I have 3 choice:
1. just write it in my CV but did not provide evidence
2.write it in my CV and provide the certificate.
3. not mention in my CV.

which is better? Thanks for your help.
