How to enter Hong Kong while in China?

I have decided to visit Hong Kong and stay there for 3 weeks at the end of my China trip. I didn't plan for the Hong Kong visit until I had already booked the ticket (from Chicago to Shanghai).

1) Is it possible to change the departure airport from Shanghai to Hong Kong? (I have American Airline ticket.)

2) Where do I get my entry permit to Hong Kong? Here in the Chinese Consulate or while in China?

I have AP.
