Please help me on NIW RFE, Thanksssssss

一年前file 140 時研究方向是diabetes，現在收到RFE,但是研究方向是heart disease。請問新準備的recommendation letter 該怎麽寫。是隻寫diabetes，還是diabetes和heart disease都寫。Thanks.
