Questions about [離境＂激活＂簽證]

PJIANG 大師, after reading your blog, I have the following questions.

1. What is "激活＂簽證 in English?

2. Why [離境＂激活＂簽證是將你在境內的身份違例一筆勾銷，不再對你將來調整身份有任何的影響]?
Do you mean that I-485 processing only focuses on applicant's status after the most resent re-entry into US, no matter if he/she has unlawful stay over 6 months or not before [離境＂激活＂簽證]?

I heard people got rejection of I-485 due to the unlawful stay over 6 months. Is it because they didn't [離境＂激活＂簽證]? Is this record from "Interagency Border Inspection Services (IBIS) check"?

3. If immigration law says [六個月以上的身份違例會造成你三年內不可入境，超過一年的話是十年不得入境。], is it conflict with the above not affecting adjust status (I-485)?
